Patient Bill of Rights
The ONYX Spine & Orthopedics Patient Bill of Rights protects you when you receive care at our clinics or surgical facilities. We believe that concern for every patient is linked closely to the delivery of safe, effective medical treatment. ONYX provides advanced facilities, surgical care, and professional expertise. You, the patient, make it possible for us to deliver compassionate care and improve outcomes for our communities.
Your rights and safety are protected by procedures that ensure you are aware of your medical choices, understand any risks or benefits, and can make informed decisions about your care. The list below summarizes your rights as a patient at ONYX Spine & Orthopedics.

You have the right:
- To safe, considerate, and respectful care, provided in a manner consistent with your beliefs.
- To be free from neglect, exploitation, and verbal, mental, physical, or sexual abuse and harassment.
- To expect that all communications and records pertaining to your care will be treated as confidential, to the extent permitted by law.
- To know the physician responsible for coordinating your care at ONYX Spine & Orthopedics.
- To receive complete information about your diagnosis, treatment, and prognosis from your physician, in terms that are easily understood. If it is medically inadvisable to give such information to you, it will be given to a legally authorized representative.
- To receive information necessary to give informed consent prior to any procedure or treatment, including a description of the procedure or treatment, potential risks or benefits, the probable duration of any incapacitation, and any alternatives. Exceptions may be made in the case of an emergency.
- To make decisions about your health care in advance through advance directives, and to expect that ONYX Spine & Orthopedics will honor these directives to the extent permitted by law.
- To receive routine services when hospitalized or treated at an ONYX facility. Complicating chronic conditions will be noted, reported to you, and treated as necessary, without assumption of long-term responsibility for their management.
- To know in advance what appointment times and physicians are available and where to go for continuity of care.
- To receive appropriate assessment of, and treatment for, pain.
- To refuse treatment to the extent permitted by law, and to be informed of the medical consequences of this decision. If refusal would jeopardize your health, you have the right to remain under ONYX care until discharge or transfer is medically advisable.
- To be transferred to another facility when your treatment at ONYX is terminated, if appropriate.
- To expect that a medical summary from ONYX will be sent to your referring physician.
- To designate additional physicians or organizations at any time to receive medical updates.
To voice concerns or file a grievance regarding your care, and to expect a prompt response without fear of reprisal.
