Advance Directives
Patients have the right under state law to execute advance directives, such as a Living Will or a Durable Power of Attorney for Health Care Decisions. ONYX Spine & Orthopedics can provide you with official state advance directive forms upon request.
If you have already completed an advance directive, please bring your most recent copy with you on the day of your procedure. As a patient of ONYX Spine & Orthopedics, you have the right to make informed decisions regarding your care. These include the right to consent, to refuse, and to withdraw consent for medical procedures.
Please note: It is the policy of ONYX Spine & Orthopedics to provide resuscitative efforts for all patients until care can be safely transferred to an acute care facility. If you have provided a copy of your advance directive, those documents will accompany you upon transfer.
